H1N1 vaccination clinic in Appleton

APPLETON - Appleton health officials have scheduled an H1N1 vaccination clinic for Tuesday, November 10 from 11:00 a.m. to 7:00 p.m. at the Northland Mall. Only the following individuals are eligible to attend this clinic:

• Pregnant women

• Persons who live with or provide care for infants less than six months old

• Healthcare and emergency medical services personnel who have direct contact with patients or infectious material

• Children aged 6 months to 4 years

• Children and adolescents aged 5-18 years who have medical conditions that place them at higher risk for influenza-related complications.

Children under the age of 18 must be accompanied by an adult, and the parent must sign the consent form. Click here to print off a copy of the form.
