1. Contest Introduction
WLUK FOX 11 has teamed up with Anytime Fitness to offer an opportunity to win 28 annual memberships to Anytime Fitness and five 11 month Anytime Fitness programs supervised by a personal trainer.

2. Contest Description
• 28 viewers will be selected via a random drawing to receive an annual membership to the Anytime Fitness Center nearest their home.
• 5 of the 28 winners will be selected by a team of FOX 11 and Anytime Fitness Judges to participate in “Get Fit 2010”, a year long fitness campaign.
• The five “Get Fit 2010” participants will be provided access to an Anytime Fitness personal trainer from February through December.
• The five participants will also receive a $200 gift pack from GNC and unlimited tanning during 2010 from Anytime Fitness.
• Each of the five “Get fit 2010” participants will be asked to:
- Set 2010 fitness goals and work with a personal trainer to accomplish those goals.
- Be available for monthly progress reports, broadcast on Good Day Wisconsin and placed on fox11online.com.
- Post text and video updates on fox11online.com
- FOX 11 will provide a flip cam and training for the video updates.

3. Description of Prizes
• 28 - One year membership to Anytime Fitness.
• 5 - Fitness campaigns designed and supervised by a personal trainer.
• 5 - $200 gift pack from GNC.
• There is no substitution, transfer, or cash equivalent for prizes, except that the Station may, at its sole discretion, substitute prizes of comparable value or cash. There will be no returns for cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.

4. How to Enter
• Viewers will be able to enter online here, or via standard mail.
- Entry information will include name, age and contact information.
• Contestants can enter online at fox11online.com. Click on the Good Day Wisconsin page then click on the Anytime Fitness icon.
• Deadline for online entries is 5pm, January 28, 2009.
• Drop off a postcard or standard sized envelope with name, address, age and telephone number at FOX 11’s studio, 787 Lombardi Avenue, Green Bay, Wisconsin.
• Mail a postcard or standard sized envelope to WLUK FOX 11, PO BOX 19011 Green Bay, WI 54304.
• Random drawings begin January 4, 2010 and ends January 30, 2010.
• The five “Get Fit 2010” participants selected for the 2010 fitness campaign will be notified by February 5, 2010.
• Content regarding goal setting and progress will begin by mid-February.
- Only one entry per mailing is permitted. Each entry must have its own stamp. No boxes or packages will be accepted.
- Only one entry per day can be dropped off at WLUK studio.
- Postcards, standard and legal sized envelopes will be accepted. Entries in envelopes larger then legal or standard size may be disqualified.
- Only one online entry per valid email address permitted. Users will be asked to complete three questions regarding their fitness goals before submitting their online entry. You may enter again using a different, valid email address.

5. Selecting a Winner
• Each day beginning January 4, 2010 and ending January 30, 2010 a winner will be randomly selected from legal entries received.
- The random selection will rotate daily from online entries to regular mail entries based on volume. Example: Monday = regular mail, Tuesday = regular mail, Wednesday = email, etc.
- WLUK FOX 11 is not responsible for lost, stolen, mangled, misdelivered, illegible, incomplete or late entries.
• The five grand prize winners will be chosen from the 28 daily winners.
- The 28 daily winners will be asked to email a photo and brief description of their goals. (Weight loss, greater stamina, run a marathon, lower their blood pressure etc)
- Some of the daily winners may be asked to attend a brief interview at FOX 11.
- Each of the five grand prize winners will be asked to provide a physician's note clearing them for the 11 month training.
- A team of judges from FOX 11 and Anytime Fitness will make the selections.
- Registering for the contest does not automatically qualify you for the drawing.
- The odds of winning vary depending upon the number of eligible entries received.

6. Winner Notification
• Each winner will be announced on Good Day Wisconsin and contacted via telephone.
• A list of all winners will also be posted at www.fox11online.com.

7. Eligibility Restrictions
• The contests are open to all WLUK FOX 11 viewers who are 18 years of age or older. Employees of WLUK FOX 11, its licensee, its corporate parents, subsidiaries, and affiliates, participating sponsors/promotional partners (GNC & Anytime Fitness), other television stations in the Green Bay Television designated market area and the members of their immediate families are ineligible to participate or win. Subject to all applicable federal, state, and local laws and regulations. Void where prohibited.
• Only one qualifier per household is permitted in the Get Fit In 2010 contest.
• Only one winner per household during the call-in contest.
• All Winners must sign a participant release and show proof of residence.
• Decisions of the Station management with respect to the contest are final.
• Each qualifier must execute and return any required affidavit of eligibility and/or liability/publicity release within One (1) day of notification attempt or prize will be forfeited and an alternate winner may be chosen by random drawing of eligible entries. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, potential winner forfeits prize.

8. Conditions
• Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners may be required to sign an IRS Form W-9 or the equivalent.
• By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
• Prior to awarding any prize or prize certificate, the Station or Anytime Fitness in their sole discretion may require contest winners (and their travel companions, if any) to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
• The Station reserves the right to make changes in the rules of the contest, including the substitution of a prize or equivalent value, which will become effective upon announcement.
• Failure to comply with the contest rules may result in an entrant’s disqualification solely at the discretion of the Station.
• The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or in the announcement of a prize.
• Copies of the written contest rules and a list of winners (when complete) are available during regular business hours at the main studio of WLUK FOX 11, 787 Lombardi Ave., Green Bay, WI 54304 or at www.fox11online.com.
• Entrants agree to be bound by the terms of these official rules and consent to use of their contact information for future communication including Anytime Fitness advertising, promotions, surveys, contest/sweepstakes and other similar communications without compensation or permission, unless prohibited by law.
